Use the patient’s details exactly
Keep the patient name exactly as it appears on the prescription, together with the medicine name and strength. If you are submitting on someone else’s behalf, say so clearly and use a contact method the family can monitor.
Separate treatment questions from access questions
The treating team answers questions about suitability, dose and side effects. An access request can only address whether information or a lawful route may be available for the prescribed medicine.
Keep the next decision with the family and clinician
Even when a case is eligible for further discussion, take time to compare the written details with the prescription and ask the treating team any clinical question before deciding what to do.
